Congestive Heart Failure – Cardiac Rehab

- Review a brief history of cardiac rehabilitation services and the evidence-based criteria the Centers for Medicare & Medicaid services (CMS) uses for coverage and payment of cardiac rehabilitation services for people with Medicare with stable chronic congestive heart failure (CHF).
- Review results of exploratory analyses comparing differences in statewide hospital readmission outcomes between CHF-diagnosed Medicare fee-for-service (FFS) patients that receive cardiac services to those that do not receive these services.
- Gain a deeper understanding of the phases of cardiac rehabilitation from the perspective of a service provider.
- Recognize the role of the healthcare provider in cardiac rehab referral and enrollment.
